Hello,
I have document provided by a client containing check boxes (sample below). We need something that will prompt the user after hitting Preview, that will display all the below check boxes and wording, will allow them to check one or many of the check boxes, and then insert what they have checked within the document.

 Assessments – Outstanding supplemental assessments.
 Delinquent Taxes – Prior year’s unpaid property taxes.
 HOA Lien – Lien against the property by the Homeowner’s Association for unpaid dues.
 Notice of Comm – Notice of Commencement
 Municipal Lien – Lien against property for unpaid municipal fees, e.g. water or sidewalk maintenance.
 HOA – Title report indicates the property is subject to a homeowners association.
 Taxes Due – Property taxes are currently due and payable
 Other – ____________

Please let me know any existing solutions or ideas. Thank you.
