Automation when Endorsements are added to a file

Hi,
I am trying to get a note to appear each time a new endorsement is added to a file. I tried setting this up through the automation processes. However, it is only recognizing the first endorsement in the file, and the note does not appear for subsequent endorsements added. Is there a way to have it come up for all endorsements that are added to a file?
