Prompt with multiple check boxes

Hi everyone
I am trying to display a prompt with multiple checkboxes like a selection list because I want to have the user be able to select the exceptions they want printed on a document, when I try to use the FOREACH ASK it only displays a field code and I want to have some sort of explanation what the exception is because not all of our exceptions have a field code so I wanted to aggregate them myself by displaying the first few words from the text as the field that is displayed at the screen with a checkbox is this functionality possible?
